• Thank you for considering UT Southwestern Medical Center Department of Anesthesiology and Pain Management for your training. Five of our eight Department of Anesthesiology and Pain Management Fellowship programs are ACGME accredited one-year training programs. Application files will be reviewed only after receipt of all required materials. The Fellowship Review Committee initiates interview requests, and not all applicants are granted interviews. An on-site interview is required before consideration for acceptance.

    We will begin accepting applications for the 2022-2023 academic year on January 1, 2021. Please see application deadlines by program below.

    Step 1: Upload the following supporting documents through this link: Submit Fellowship Documents.

    • UTSW Anesthesiology Fellowship Application (PDF) *Neuro and Pedi Cardiac * or Common Application (PDF) *Regional Only*
    • A recent digital photo.
    • A current curriculum vitae. Please include information regarding any hospital, graduate school or medical research experience and medical school or postgraduate honors.
    • A copy of your USMLE or Comlex scores Steps 1, 2 & 3.
    • A copy of all of your In-Training Examinations results.
    • A copy of current ECFMG certificate (if applicable). 
    • International Medical Graduates - At this time, we are not accepting or sponsoring any training visa(s).

    Step 2: Request the following letters of reference to be uploaded by your letter writers through this link: Submit Letter of Reference

    A minimum of three letters of reference:
    • One letter must be from the Department Chair or Residency Program Director of the program where you completed residency training
    • Two letters from other current references and/or attending physicians and/or teaching faculty
